Description:

You will be part of the core RDC team comprising an RDC Advanced Nurse Practitioner (ANP), RDC Clinical lead, and an administrative support worker. The RDC team will be primarily supported by all consultant radiologists, oncologists and haematologists in addition to Trust consultants whose expertise is required to manage RDC patients. Referrals to RDC will be received directly from primary care and are triaged by the RDC ANP and/or GPwSI Oncology, according to the established referral criteria for RDC. Only those referrals deemed appropriate for RDC are accepted.
Referrals will be tracked on the Trust Cancer Wait Times databases (InfoFlex). Following appropriate triage, RDC-appropriate patients will be seen by the RDC GPwSI Oncology and RDC ANP. Relevant diagnostics will be organised in a time appropriate manner with follow up of these results being facilitated by RDC ANP and GPwSI Oncology in conjunction with the RDC administrator. The RDC clinic will run for 2 days per week.
All clinic documentation will be on the Trust iCM (Trust electronic management system) and iPM (Trust Patient management system). The responsibility of ESTH RDC clinic is to appropriately investigate patients with symptoms or signs concerning for cancer. This includes, but is not limited to, organising and ensuring follow up of investigations such as CT, MRI and PET scans, radiology guided biopsies, and endoscopies. The clinic will also organize referral to the relevant tumour-specific multidisciplinary team (MDT), as needed.
Patients in whom a cancer diagnosis is made will be transferred to the relevant MDT, whilst those in whom cancer is excluded will be referred to the relevant specialist service at ESTH and/or referred back to the GP. Initial diagnosis and management of the patients non-oncology symptoms will be undertaken by the GPwSI, but longer-term management or follow up of the patients cancer or non-cancer diagnoses is not within the remit of RDC. Patients will be discharged from RDC once cancer has been diagnosed or excluded to the appropriate team either within primary or secondary care setting
