Description:

PHL is an established and expanding healthcare organization dedicated to providing a range of urgent and primary healthcare services across Hampshire. We are currently seeking a General Practitioner (GP) to join our team in Havant and Waterlooville, to lead the delivery of Enhanced Access appointment provision for the local Primary Care Networks (PCNs).

SKILLS AND EXPERIENCE:

  • Experienced doctor with GP accreditation.
  • Current registration with GMC and NHS performers list (with no conditions preventing working or restrictions on practice).
  • Experience working in primary care and urgent care environments.
  • Record of ongoing CPD and learning.
  • MRCGP examination or equivalent experience.
  • Sound knowledge of QOF, LCS, DES, CQUINs, and appropriate coding.
  • Strong clinical skills in the assessment, diagnosis, and management of acute primary care presentations.

Responsibilities:

ABOUT THE ROLE:

As a GP with PHL, you will be responsible for delivering safe and effective primary care appointments in accordance with GP Improved Access hub criteria, with full access to patient medical records. Our team of clinicians offers evening and weekend appointments to local patients referred by their own GP surgery and 111.

KEY RESPONSIBILITIES:

  • Lead the delivery of Enhanced Access appointments for local PCNs.
  • Provide clinical assessment, review, and examination of patients, requesting appropriate investigations (e.g., x-rays, scans, blood tests).
  • Develop and agree on diagnosis and management plans, making credible onward referrals to specialists when necessary.
  • Work with a multi-disciplinary team to ensure appropriate care is delivered in the right place, at the right time.
  • Assess, investigate, diagnose, and treat patients presenting with primary care needs.
  • Ensure appropriate coding and support the delivery of QOF, LCS, DES, and CQUINs.
  • Refer to secondary care using appropriate referral forms and mechanisms.
  • Provide emergency care while waiting for emergency patient transfer when necessary.
  • Offer advice and support to colleagues and the multi-disciplinary team.
  • Maintain continued education through attendance at courses and/or study days to meet professional development requirements.
  • Participate in annual appraisals and complete a personal development plan.
