Description:

An exciting opportunity has arisen for an experienced nurse to join the Surgical Assessment Unit Team as a GP Liaison Nurse. The candidate needs to have the ability to work autonomously and will have the responsibility of liaising directly with GPs and other external agencies. they will need to possess an extensive awareness of the SAU pathways and community services in order to signpost ensuring that patients are in the right place at the right time.

1. Co-ordinate timely access for all emergency GP referred patients to the hospital, contributing to the clinical prioritisation of emergency patients, deciding on appropriate admission arrangements to match the activity within the Emergency Department, SAU and overall demand of the trust.
2. A knowledgeable practitioner, operating agreed procedures for the safe and effective use of the trusts capacity within ED, MAU, SAU and the Trust capacity in emergency/fast track/ambulatory clinics; identifying issues which prevent the safe and effective management of emergency Gp referrals, planning, negotiating, and agreeing access. Must have a robust knowledge of all speciality care pathways and promote within the role to the Community GPs, ensuring there implementation.
3. Effectively communicate information regarding clinical decisions, policy and care pathways to patients/carers and the multi-professional team including situations where difficulties incomprehension occurs.

5.Early identification of vulnerable patients referred by Gps and community teams and ensure there is appropriate support in place as soon as possible to help smooth the patient’s length of stay
6. Working with WMAS SDEC to continue to promote the service to help the patient be in the right place at the right time
