Description:

We are seeking a dedicated Gynaecology Oncology Patient Pathway Facilitator to join our team. In this role, you will take responsibility for proactively tracking patients on the cancer pathway, ensuring that they receive the appropriate care and support from the moment they enter the Trust.
As a Gynaecology Oncology Patient Pathway Facilitator, your primary responsibility is to guide patients through their care pathway, from referral to treatment, ensuring adherence to pathway steps and timescales. You will utilise hospital IT systems such as eTrack, Epro, ICE, and Somerset to track patients at every stage of their journey and ensure diagnostic tests and treatments are completed within targets. Additionally, you will play a crucial role in reducing patient cancellations and non-attendance by emphasising the importance of timely appointments and facilitating administrative transfers to tertiary centres as needed.
Your proactive coordination of appointments and escalation of diagnostic appointments for target patients are essential in identifying and addressing delays in the patient pathway. You will collaborate closely with the wider team to ensure targets are achieved, including flagging results to clinicians and ensuring quick turnaround of result letters to patients. Through your dedication and attention to detail, you will contribute to the seamless and efficient delivery of cancer care services.
