The Health Care Assistant will be responsible for providing skilled nursing care to the residents of Tower Hamlets, promoting the health of the population and reducing the effects of ill health and disability and maximising the individuals potential for independence. S/he will work closely with General Practices, Social Services and other statutory and non-statutory services to meet the needs of the local population using public health information to support service development. You will be part of the team who ensures all new admissions to the Community Nursing services have a clinically robust assessment and patient focused care plan.

To work as a member of the Community Health Team, providing rehabilitation, nursing care and to patients in their homes, community care settings and/or in a group setting.
The Community Health Team (CHT) will manage via a single locality/Network caseload all patients along a step up/down model which will incorporate care from a two hour response, short interventions, rehabilitation, through to the management of Long term conditions and end of life care.
The aim of the team is to maximise the independence, health, and well-being of adults in order to enable them to return to independent or supported living in the community wherever possible.
The CHT works with adults in the community who have various needs such as long term neurological conditions, recent surgery, risk of falls, palliative care, wound care needs, reduced physical and functional ability. CHT is a 7 day service operating 8am -8pm.
The team is also supported by the extended primary care team that consist of Physiotherapist, Occupational therapist, Care Navigators, and Admin. The nurses component has the emphasis on working with the senior nurse working with families to promote independent living.
The nursing team has 1 Band 8A, 2 Band 7, 5 Band 6, 2 making up the senior nursing team. They are supported by 6 band 5 nurses and 2 Band 4 SHCA and 4 HCA with nursing skills. This gives good peer support and development to the team of nurses. The integrated team across social services, voluntary sector and health input make up the team to plan the patient focus care plan.
