POSITION SUMMARY:
Schedule - 9:00AM 5:30PM EST, 8 hour shift, every other weekend, 4 holidays per year. Schedule to be determined upon hire
At the 24-Hour Nurse Line the Health Coach Consultant utilizes a collaborative process of assessment, planning, implementation and evaluation, to engage, educate, and promote/influence members decisions related to achieving and maintaining optimal health status.
- Facilitates effective health care decision-making.
- Improves the quality of care.
- Improves member satisfaction with their health care plan.
- Facilitates appropriate use of health care resources.
- Provides up-to-date health care information to help facilitate members’ understanding of their health problems and possible options.
- Provides assistance by clarifying available options for the member’s particular health problem or issue.
- Helps members actively and knowledgeably participate with their provider in their own health care decision-making.
- Promotes healthy lifestyles to help lower personal health risks, preventing illness.
- Awareness of the member’s available health and wellness programs, and makes referrals to these programs as appropriate.
REQUIRED QUALIFICATIONS:
- Must have active, current and unrestricted RN compact licensure in state of residence
- Must reside in Compact licensure state
- 3+ years of clinical experience required, specifically in: Disease Management, Emergency Room, Critical Care, Medical/Surgical Care, Oncology.
1 year proficiency in Microsoft Office Suite (including Outlook and Microsoft Word).
Must be able to work hours required: Start time will be between 9:00AM -5:30PM EST, 8 hour shift, every other weekend, 4 holidays per year.
Schedule to be determined upon hire
PREFERRED QUALIFICATIONS:
- Managed Care experience preferred.
- Previous coaching experience preferred.
- Model candidates have telephone triage nursing experience.
EDUCATION:
Associate Degree required.
BSN preferred
