Description:
- Suffolk County Council – Hadleigh IP7 6RJ and Sudbury CO10 2DZ (Community based)
- £38,925 - £44,305 per annum (pro rata if part-time)
- Post 1: 18.5 hours per week - Hadleigh

Responsibilities:
You will:
- lead delivery of the Healthy Child Programme for ages 0-5
- conduct assessments and care planning
- evaluate outcomes and support service improvement
- champion health promotion and reduce inequalities
- provide universal health reviews and targeted support to families
- safeguard children through collaboration and assessments
- be the key contact and case holder for families
- maintain NMC registration and work to The Code.
