Description:
To manage, lead and co-ordinate the implementation of the health and wellbeing model within the Kent of Prisons. (model attached).
To provide and develop a specialised programme of care, or care packages in the Kent Prison Cluster that will address the prisoner’s presenting vulnerabilities:
Primary care needs and long-term conditions
Mental health needs
Substance misuse
To provide safe and effective care pathways for all offenders identified through the single point of assessment as needing care co-ordination.
Working alongside the responsible prison clinical team, the post-holder will arrange and organise meetings that will support the health and wellbeing model. These meetings include multidisciplinary meetings, case conferences, referral/discharges, and monthly monitoring meetings.
Key Tasks and Responsibilities
Provide co-ordination of and participate in the health and wellbeing working group and provide expertise and support where requested.
Maintain accurate and timely care co-ordination activity, information, and data on an electronic data tracker system.
The data collected must identify all prisoners that are being care ordinated. The referrals made to each department based on the needs of the patient should be recorded.
Monitoring the response of each service should be captured and challenged appropriately and sensitively based on the clinical need of the patient.
Be responsible for monitoring the care co-ordination of offenders including referrals to internal and external healthcare services.
