About us
Murrumbidgee Primary Health Network (MPHN) is a not-for-profit organisation servicing more than
252,358 people, living across 508 communities on a land mass of 126,124 square kilometres. It is
one of 31 local and independent PHNs established nationally to support the primary healthcare
sector and improve health outcomes, particularly for those at-risk populations.
MPHN achieves this by understanding the region’s health needs to inform:
- funding services to meet population health needs;

Position Description
Position title: HealthPathways Manager
Agreement and firsthealth Limited Enterprise Agreement
classification: Level 5
Status: Full time 37.5 hours per week
Location: Murrumbidgee Region
Reports to: Senior Manager Integration
Purpose
The HealthPathways Manager is responsible for providing effective leadership and management of
the HealthPathways program, while ensuring the team contribute effectively to the strategic and
operational objectives of the program. The HealthPathways Manager will also be responsible for
developing and maintaining collaborative relationships with key stakeholders in the program and
embedded participation across the region.
Key accountabilities:
- Provide effective leadership, management and direction to the HealthPathways team
- Contribute to the development of strategic and operational plans for the HealthPathways
program with the identification of key performance indicators for the program
- Coordinate the development, maintenance and continual quality improvement of
HealthPathways ensuring it remains relevant and useful to health professionals and
collaborative partners
- Develop and foster effective engagement strategies for consultation with key stakeholders and
other health service providers for the purpose of developing HealthPathways
- Actively use the pathways administration system (DOT) to manage pathway development, triage
and allocation of feedback, produce reports and share pathways content within the
HealthPathways community.
- Support and assist with writing content for HealthPathways in liaison with Clinical Editors,
Subject Matter Experts (SMEs) and Streamliners. Develop budget proposals that reflect the
needs of the program and maintain financial oversight of the approved budgets
- Assist with the development and implementation of eHealth processes that relate to
HealthPathways to improve integration opportunities across the region, including eReferrals
- Facilitate clinical working groups to ensure positive outcomes are achieved and documented in
the pathways development process
- Assist in the planning, implementation and evaluation of clinical redesign strategies as identified
by the clinical workgroups.
- Produce reports as required relating to the program’s performance and against KPI targets
- Develop and oversee educational and promotional activities to promote HealthPathways to
internal staff and external stakeholders
- Actively support general practices in the implementation and uptake of HealthPathways
focussing on sustainable system change within primary care.
- Undertake research of local referral paths to support the building of HealthPathways content
- Undertake continuing education as required to ensure job skills and knowledge remain current
- Contribute to the MPHN’s Quality Management System including identify opportunities to
improve policies, procedures and processes.
- Work Health and Safety responsibility including identifying, assessing, eliminating/controlling
and reporting hazards and risks within the workplace, to the extent of delegated authority for
the role.
- Attend scheduled meetings and work-related functions within and outside usual work hours
including travel to other regions where overnight stays may be required.
