Description:
The Health Mentor role involves the provision of early intervention with Children, Young People and Families where there is a potential risk to health and wellbeing. This role will focus on key lifestyle issues that have an adverse impact on health and wellbeing-
- Alcohol/Drugs
- Smoking
- Healthy Weight
- Oral Health
- Sexual Health including teenage conception
- Emotional Health and Wellbeing
- School Readiness and transition to Primary School
- School transition to high school
- Transition to Adult Services
To provide one to one or small group support, advice and guidance to specifically targeted Children and Young People aged 5-19 (Particularly those from deprived areas and those with identified health risk behaviours)
To assess readiness to change behaviour and offer health care plans based on need.
To support Children, Young People and their Families to manage behaviour change.
To work closely with School Health, family Hubs, Schools and other agencies
