An exciting opportunity has arisen in the Cardiology service at Tameside and Glossop integrated Care NH Foundation Trust. We are looking to recruit a Band 7 heart failure specialist nurse who is dynamic and experienced to support the service site wide with support from the wider heart failure MDT and Heart failure Cardiologists. This team is integrated, in and out-reaching to patients referred from General Practice with heart failure to support from across the 5 neighborhoods across Tameside and Glossop. This service provides expert advice and treatment to patients with heart failure with a clear plan to improve the delivery of evidenced based care to this group of patients.

THE POST HOLDER WILL: -

Deliver Integrated hospital / community specialist advice and support to patients, carers and other professionals with regard to chronic heart failure.
Manage patient care and adjust treatments within local and national guidelines and evidence base.
To lead and develop local action plans to meet the supportive and palliative care agenda.
Work closely with the Multidisciplinary team to deliver effective management of patients suffering chronic heart failure.
Be responsible for education and training of the multi-disciplinary team in the management of chronic heart failure.
Provide support across Tameside and Glossop for heart failure education and audit
Responsible for data collection and completion in order to meet local and national audit requirements for patients suffering chronic heart failure.
Act in accordance with the NMC Code of Professional Conduct
