Highly Specialist Occupational Therapy Team Lead

The post holder will work in collaboration with the multi-disciplinary team and other partners to ensure care delivery is evidence-based, needs-led, recovery focused, effective and evidence-based. Work with Service Managers and professional manager to ensure equity and accessibility of service provision across Cwm Taf Morgannwg University Health Board. Carry out highly specialist assessment which informs multi-disciplinary team care plans (e.g. contributes to diagnostic processes; assists with risk assessment and the development of risk management plans/risk formulation; highlighting or refuting the need for specialist placement; and identifies appropriate package of care).
Share outcomes of OT assessment to determine whether rehabilitative/ recovery focused or compensatory approaches should be adopted by the multi-disciplinary team, ensuring assessment and intervention outcomes are available to the multi-disciplinary team in a timely manner, using excellent time management skills. Deliver highly specialist occupational therapy assessment and interventions to a referred caseload. Formulate individual occupational therapy intervention plans for individuals on own caseload across Older People’s Mental Health services, based on highly specialist knowledge of relevant evidence based practice and using clinical assessment, advanced clinical reasoning skills and knowledge of interventions
