Description:

An exciting opportunity has arisen for an enthusiastic and motivated physiotherapist to join our expanding Admission Avoidance and Discharge Service (AADS) at East London NHS Foundation Trust.
This role will provide a successful applicant with the opportunity to take a lead role in Rapid Response and Intermediate Care including the Falls Pick Up Service. The highly specialist role is based in Mile End Hospital in the borough of Tower Hamlets and is part of the Admission Avoidance & Discharge Service (AADS) which consists of a team of multidisciplinary professionals that work across the community and hospital to facilitate early discharge and prevent unnecessary hospital admission.
We are looking for a highly motivated, innovative, and flexible senior physiotherapist to join our team in working with patients across a variety of clinical areas. Applicants must have a strong clinical skills as well as the ability to manage complex cases independently, providing a comprehensive therapy assessment and intervention, evidencing problem solving and clinical reasoning skills in line with evidence-base practice.
The Admission Avoidance and Discharge Service in the community operates a seven day service, covering core hours between 8am and 8pm with shift of 8-4pm and 12-8pm. You will need to be a versatile, adaptable team player who is prepared to work across a 7 day rota.
The post holder will be required to work with partners across the health and social care interface to ensure a joined up and integrated approach is adopted to the provision of services. All staff are expected to work across traditional boundaries of both community and acute services, to deliver truly holistic care.
To be professionally accountable for all aspects of own work, including management of highly complex patients in your care to prevent relapses/injury and identify risks for admission. To ensure a high standard of clinical care for patients under your management, and support more junior staff to do likewise.
To provide rapid response, falls pick up, and specialist multidisciplinary triage in the community.
To formulate accurate physiotherapy assessment and prognosis of patients/clients presenting with a broad range of highly complex physical, cognitive and psychological conditions and disabilities by utilising interpreting and analytical skills in order to recommend the best client-centred treatment plan and develop comprehensive discharge plans in order to meet patients’ complex physical, cognitive, social and environmental needs.
To provide support in the Admission Avoidance in the Emergency Department at RLH, dependent on demand.
To accept clinical responsibility for a designated caseload of patients and organise this effectively and efficiently with regards to clinical priorities and use of time. To ensure the junior staff do likewise.
