Description:

Sutton Community Health and Care are looking for an enthusiastic innovative Band 7 Physiotherapist to complement their Care Home Support Team. The successful candidate will work in collaboration with Sutton Place to support their work through the Enhanced Health and Care Homes Programme.
The Care Home Support Team provides treatment and care for a wide range of healthcare needs for residents in care homes within the London Borough of Sutton/
The role aims to support the review of resident’s mobility regarding activities of daily living, with the formulation of plans to support the resident maintaining levels of independence where able.
The successful candidate will be required to design, implement and evaluate a standardised approach to physiotherapy delivery in Sutton care homes, focusing on promoting the use of regular exercise, appropriate movement/interactive and wellbeing strategies. The programme’s aim is to reduce the risk of falls, improve balance, avoidable physical and mental health decline.
The post holder will be required to develop and maintain effective working relationships with care home managers, GPs, nurses, healthcare assistants and programme stakeholders as well as with other members of the MDT and, therefore, will need to demonstrate a flexible approach and excellent communication skills.
The post holder will also be required to collect data to demonstrate achievement of the agreement programme outcomes, to enable the service to regularly report back to commissioners.

Responsibilities and Results
To have clinical responsibility for assessment and treatment planning for people referred to the service
Work autonomously and as part of a multidisciplinary care team
Manage a clinical caseload which comprises of adult clients with physical and cognitive difficulties within the community and/or bed based setting
Supervise junior colleagues and assistants
Assist in the overall development and management of physiotherapy across the service
Undertake clinical practice at a specialist level (including non-medical prescribing skills) and provide clinical advice and management to patients referred to the Care Home Support Team, professionals and others where appropriate
Collaborate with colleagues across own and other community teams, acute teams, GPs, social services and voluntary sector to optimise the patient’s journey, care, wellbeing and outcomes
Develop a high level of specialist clinical skill in assessment, treatment planning and implementation of agreed goal orientated intervention with residents/carers
Assess and treat own caseload of patients, who may have a highly complex presentation and maintain associated records as an autonomous practitioner
Contribute towards single assessment and trusted assessment processes
Provide comprehensive intervention to patients in a care home setting, making use of assessment and treatment facilities where appropriate
Be responsible for a clinical caseload working within the MDT ensuring residents and carers receive optimum level of therapy within available resources
Carry out multidisciplinary screening assessments and discuss outcomes with MDT members
Keep accurate up-to-date records on patient care and equipment provision, which comply with quality standards, meet legal requirements and can be shared with colleagues and carers
Provide complex written and/or verbal reports to referrers and other agencies involved in resident’s care and participate in clinical meetings as necessary
Assist in the development of physiotherapy within the service and effectively contribute towards the patient care pathway, and to develop the skills required to undertake this
Monitor the effectiveness of intervention through goal setting, outcome measurement and evaluation of practice
Contribute to the development of information to enable residents to understand and self-manage their condition
Make rapid decisions, using sound clinical reasoning skills and medical knowledge
Review evidence based literature to ensure safe and effective practice
Be competent in the use of IT including the formulation of databases for research and audit purposes
Demonstrate effective communication skills with stakeholders both orally in in writing producing reports etc., and taking a lead role in meetings and discussions.
Research and Development
Organise, develop and deliver teaching sessions for care home staff and other healthcare professionals
Act as a resource for specialist physiotherapy information, relating to mobility and wellbeing support to the primary health care team, other community groups and related interest groups
Develop evidence bases written resources and education material for use of care home staff, residents and other healthcare professionals
Identify and undertake audit and research that will enhance patient care and to present these results in report form as appropriate.
