Description:

MAIN DUTIES AND RESPONSIBILITIES Clinical and Professional 1. To be the Physiotherapist responsible for the day to day provision of appropriate Physiotherapy to the above client group. 2. To be professionally and legally responsible and accountable for all aspects of physiotherapy intervention and care delivered to clients.
To ensure a high standard of clinical care, and support less senior staff to do likewise 3. To be able to carry and prioritise your own designated clinical caseload, working as an autonomous practitioner without direct supervision. 4. To take appropriate action to deal with any emergencies identified including identifying and responding to risk or abuse situations in accordance with policies and procedures.
5. To provide specialist Physiotherapy services to adults with a Learning Disability ensuring high standards of assessment and treatment/ management utilising clinical reasoning and a wide range of treatment skills. 6. To apply a person centred model in working towards improvement in physical skills, mobility, general health and independence in accordance with the valuing people white paper.
7. To liaise and work with the wheelchair service regarding referral, assessment provision, monitoring and repair of wheelchairs and specialist seating. 8. To contribute to clients individual progress planning meetings, case conferences, review meetings and health promotion planning.
9. To monitor and evaluate intervention to measure progress and to ensure effectiveness of the intervention using appropriate outcome measures and evaluation tools. To modify physiotherapy programmes as required. 10.
Effectively communicate with clients and carers in order to maximise an individuals functional potential. 11. Carry out safe therapeutic manual handling manoeuvres. Where appropriate, following Physiotherapy assessment provide advice and conduct risk assessments for individuals involved in moving and handling clients.
12. To act as a specialist advisor for other Physiotherapists within Croydon Health services managing clients who have a Learning Disability. 13. To liaise with Physiotherapy colleagues and staff from other disciplines within and external to Croydon Health Services NHS Trust ensuring effective communication at all times in order to provide excellent patient care within the Physiotherapy Service.
14. To be aware of other clinical interventions that can be used in conjunction with Physiotherapy practice to maximise treatment benefit e.g. orthotics and special seating and work collaboratively with the other health professionals involved. 15.
To keep comprehensive, accurate and up-to-date records of assessment and intervention according to service, professional and legal standards. To ensure that more junior and unqualified staff do likewise. 16. To be fully conversant with the Mental Capacity Act and how it translates to practice.
Gain appropriate consent for treatment/ intervention and ensure that service users understand the intention to treat Where this is not possible ensure that the principles of MCA are followed and documented appropriately- ie consent discussions are fully reflected in clinical notes. 17. Providing targeted episodes of care and ensure that client and carers are aware of goals and outcomes 18. Keep fit sessions- gym sessions risk assessments 19.
Walking groups- risk and environmental assessments pertaining to clients and behaviours 20. To ensure safe and correct use of any aids and appliances used by service users- trusted assessor and to provide equipment from the community equipment service or other provider
