Home 1 st Twilight Nursing Service is looking to welcome new colleagues to their supportive and integral team . The successful candidate will assist registered nurses in providing unplanned nursing care to patients in the Milton Keynes Community outside of usual working hours. The service operates from 7pm to 2am.
In the Twilight team you will work as a Health Care Support Worker . In this role you will be working with a registered nurse to delivering the effective and efficient provision of care to aid patient’s recovery, promote independence, health and wellbeing following sudden illness or deterioration.
Work as a member of the multidisciplinary team observing, recording and reporting any changes in a patient’s condition or circumstances to registered staff.
Be familiar with the steps necessary in an emergency and be able to act accordingly.
To provide planned, delegated nursing interventions for patients in their own homes or other settings as appropriate.
To participate in shared care with other agencies / services as required.
Provide accurate feedback on patient’s care to Nursing teams.
Ensure the needs of carers and relatives are listened to and reported back to the Nurse in Charge of the shift.
Ensure clear, concise and accurate records are kept and updated in a timely manner, including the use of departmental electronic records.
To be aware of health and safety and follow risk assessments to ensure the safety of service users and staff.
To undertake relevant training as provided.
Apply knowledge/skills gained from education/training so that a high standard of care is maintained at all times.
Assist with personal care as required, promoting independence and ensuring dignity and respect in care.
Carry out observations of vital signs according to competency and under instruction from registered nurse.
To carry out pressure area care, eye care, catheter care, stoma care, personal care and collection of specimens / medicines and equipment as per care plan and in line with agreed competency .

Undertake delegated duties according to patient’s individualised care plan.
To carry out pressure area care, eye care, catheter care, stoma care, personal care and collection of specimens / medicines and equipment as per care plan and in line with agreed competency .
To obtain consent to any procedure from each patient and to proceed only when consent is obtained. Document when procedures are carried out in patient’s best interests.
Be familiar with the steps to be taken in medical emergencies and act accordingly.
To maintain accurate records of intervention on the patient’s electronic records on each shift and in line with the CNWL care record standard policy.
To use equipment appropriately as described in the patient’s care plan.
To attend in-service training, clinical supervision and team meetings as required.
To work across Home 1 st Services when there is a need.
To adhere to trust appraisal and performance review programme to identify personal development plans and training needs.
To participate in data collection and statistics as directed.
Maintain equipment, identify defects, hazards and equipment failure and notifying the nurse in charge and report defect via Datix.
Manage resources including equipment and supplies in an economical way to prevent waste, while ensuring standards pertaining to infection control and safety are met at all times.
Have respect for patient’s property.
Contribute to the maintenance and safety of designated work environment according to local policy and regulations and be aware of the emergency procedures.
