Responsibilities:

Our House Calls program is growing in the Chicago suburbs! Come join us in bringing outpatient primary care to patients in their homes.

• Cover assigned territory to care for patients in their homes
• Chronic care management
• Yearly assessments
• Weekday clinic schedule
• Work on a team with Physicians and NP’s
• Mileage reimbursement
• Excellent benefits, PTO and retirement plans
• Malpractice insurance with tail coverage included

Qualifications:

EDUCATION/TRAINING

• Graduate of an accredited nurse practitioner or physician assistant program
• Masters degree in Nursing or Physician Assistant Studies
• Current Illinois nurse practitioner or physician assistant license in good standing
• Certified as a nurse practitioner or physician assistant by an approved certifying organization.
• Prescriptive authority, Illinois Controlled Substance, DEA certification
• Current CPR certification
  Come to be part of the world-renowned University of Chicago Medicine system, while still working in community medicine
