Description:

JOB SUMMARY: The Housing Coordinator (Mercer DMHAS, COC and HUD 811 Community Housing) is responsible for the administration of all Housing Assistance Programs. Provides administrative management of Mercer County leased apartment units and leased rental units. Under the direct supervision of the Clinical Program Director of Residential; work collaboratively with the Program Supervisor, Director of Residential Services, Mercer Housing Team and Fiscal Department to ensure effective communication and shared information.

Responsibilities:

  1. Acts as the primary administrator for the Supportive Housing Programs in locating a variety of housing options for consumers to establish affordable, permanent, supportive housing in the community.
  • Establish and maintain positive working relationships with the various administrators of financial assistance programs (rental subsidies) to promote access to funding for consumers. - Provide assistance to the Clinical Program Director of Residential and/or Director of Residential Services; Program Directors and Supervisors regarding all options available for consumers to received financial assistance (rental subsidies) to establish affordable, permanent, supportive housing.
  • Act as a resource for Housing Specialists, as well as other agency Behavioral Health programs regarding the options available for affordable, permanent, supportive housing.
  • Act as the point of contact for partnering rental agents, housing developers, and property managers in securing housing and in ensuring response if problems arise.
  • Provide education and collaborate with support staff, Program Directors and Supervisors on Tenant and Landlord Rights in New Jersey to ensure that consumer tenants are aware of their responsibilities and when they are violating the terms of their lease.
  1. Act as the primary administrator of the DMHAS Rental Subsidy Program with staff from DMHAS and the Supportive Housing Connection (SHC).
  • Complete initial and renewal subsidy documents with consumers and submitting all documents to the SHC; ensure that the correct amount of subsidy is paid to the landlord/property management and provide assistance in resolving payment problems with the SHC.
  • Communicate with SHC staff to resolve any issues that may arise during the lease up or renewal process. Distribute information/notices to the program staff working with the consumers. Provide coordination for Mercer Supportive Housing; On My Own and At -Risk program; and with the Office of Housing, DMHAS. Reconcile any discrepancies with DMHAS staff to assure correct numbers of subsidies are assigned to these programs.
  • Provide assistance to Clinical Program Director, Program Supervisor and Team Leaders of Mercer PACT teams as it relates to the Housing Assistance Programs.
  1. Assist the point of contact and primary administrator of the DMHAS B.E.D.S program.
  • Provide program information to the primary administrator on a weekly basis for accuracy and updates as needed. Collect any changes or updates to be made in B.E.D.S from Housing Staff.
  • Provide information including consumer discharges on occupied units and vacancies.
  • Provide spreadsheet of updates and changes to be made in the B.E.D.S system by DMHAS staff and reconcile any discrepancies in the information in B.E.D.S.
  1. Oversight of the property management of the 3 HUD 811 Community Shared Homes in Mercer County.
  • Process all leases and rental subsidy documentation with tenants. Provide education on lease and rental subsidy to tenants or staff as needed.
  • Act as point of contact for the Landlord Registration Office of Mercer COT; NJ Department of Community Affairs; NJHMFA and the Supportive Housing Connection, ensuring that all fees are paid in a timely manner and inspections are completed as required.
  • Hold regular Tenant meetings at the property with the Program Leadership and Housing Specialists that provide services to the tenants.
  • Collaborate with Clinical Director and/or Program Supervisor and Housing Specialists in regards to any lease violations and develop plans to prevent eviction action with the tenant and staff.
  • Will serve as EIV/TRACS/REAC point person.
  • Responsible for admissions and quarterly inspections to all HUD 811 community shared homes.
  • Serves as contact point person between CCDOT Mercer and PMCS.
  • Responsible for all required and mandated reporting for these elements.
  1. Management of leases for CSS consumers that are leased throughout Mercer County, NJ.
  • Act as one of the primary point of contacts to landlords and property managers for assigned units.
  • Assist in resolving payment issues of rent and/or subsidies for leased units.
  • Track and process lease renewals for those units leased by CCDoT. Send leases to CAS for review and signature of Executive Director.
  • Process all leases with occupants/tenants.
  • Report any issues regarding tenant behaviors or lease violations to the Clinical Program Director and/or Director of Residential Services
  • Collaborate with Clinical Director and Housing Specialist in regards to any lease violations and develop plans to prevent eviction action with tenant and staff.
  • Act as primary point of contact for property maintenance, repairs for Mercer COC’s and HUD 811 properties.
  1. Assist the Clinical Program Director and other program leadership in attendance to internal and external meetings that include, but are not limited to: Mercer County Continuum of Care.

  2. Assist the Clinical Program Director, Program Supervisor and Lead Housing Specialist in communicating with the Department of Community Affairs and HUD with Project Managers for COC vouchers for chronically homeless individuals in Mercer County.

  • Enter and update program participants’ information in the HMIS program. Assist to Insure quality of date entered into HMIS per HUD guidelines.
  • Complete and submit annual grant renewal documents as required to the project manager of the COC vouchers with Mercer COT.
  • Complete and submit annual documentation of service and dollar amounts to project manager of the Mercer COC with Mercer COT.
  • Announce vacant subsidies to CCDoT programs for referrals; conduct preliminary eligibility review and forward eligible participant information to project manager of the HUD 811 and Mercer COC program.
  • Contact CEASE for referral process for eligible individuals on the Waiting List for housing based on the County’s Comprehensive Assessment process.
  • Complete and submit applications for eligible individuals to the project manager of Mercer COC’s.
  1. Responsible to provide requested information in HMIS for Mercer – CSS Program.
  • Enter required information into HMIS for each program participant of Mercer County Leasing and Trenton Permanent Housing as required by HUD.
  • Maintain accuracy of information in HMIS.
  1. Assist the Clinical Director of CSS and Residential Services and/or Director of Residential Services, as needed, in responding to new funding opportunities and service initiatives for Mercer Behavioral Health Services- Residential Programs.
  2. Attend ongoing COC and HUD trainings instructed by the Clinical Director of CSS and Residential Services.
  3. Any additional duties as assigned by Clinical Director of CSS and Residential Services and/or Director of Residential Services as applicable
