POSITION SUMMARY:

As a part of a multidisciplinary Residential Services Team, the Housing Navigation Coordinator role is to create initial contact and rapport with Housing Navigation Center (HNC) clients. Housing Navigation Coordinator receive, greet and check clients into the HNC. Housing Navigation Coordinators provide clients with information pertaining to housing information and resources. Housing Navigation Coordinators act as a liaison between clients and case management. Housing Navigation Coordinators are able to facilitate assessments such as but not limited to shelter enrollment and diversion service assessments. Utilizing concepts of Housing First and Harm Reduction modalities the Housing Navigation Coordinators provides supportive services for clients. Housing Navigators play a crucial role in the success of the client moving from homelessness to home. Working hand and hand with Case Management Housing Navigation Coordinators advocate for clients and their needs. They also track outcomes and completes individual progress notes.

KNOWLEDGE, SKILLS AND ABILITIES:

  • Must be computer literate and possess effective written and verbal communication skills.
  • Ability to work independently or with a group.
  • Must have a strong sense of focus and must be task-oriented and non-judgmental with a clear sense of personal boundaries.
  • Must have a respect for confidentiality involving both clients and employees.
  • Ability to work in a variety of settings with culturally diverse groups. Ability to be culturally sensitive and appropriate.
  • Must be able to manage personal appointments and schedule, prioritize work and meet necessary deadlines.

QUALIFICATIONS:

  • Prior full time professional level casework experience in a social service agency preferred.
  • Prior experience with homeless population required.
  • Experience with chronically homeless men preferred.
  • Flexible schedule to work after hours and weekends.
  • Regular schedule will include either Saturday or Sunday each week.

PHYSICAL REQUIREMENTS:

  • Primarily works in the Housing Navigation Center, but will also work throughout the agency and off site.
  • Ability to regularly communicate with clients, co-workers and supervisors.
  • Ability to operate and use office equipment, especially computer, keyboard and mouse.
  • Ability to continuously sit and/or stand.
  • Ability to walk continuously if needed and climb stairs as needed.
  • May occasionally lift up to 25 pounds.

Responsibilities:

  • Greet clients
  • Check clients into the Housing Navigation Center
  • Facilitate Night Shelter Enrollment
  • Work closely with Case Management to create seamless transition from the homelessness to housing
  • Maintain awareness of potential resources in the community and make proper referrals.
  • Work with Case Manager’s to asses and identify client’s eligibility to various housing, employment, substance use, nutritional assistance or other applicable programs available in the community and provide appropriate program guidelines to clients.
  • Complete a range of required program related documentation utilizing a range of computer programs and data collection systems.
  • Protect all confidential information, company property and electronic data. Comply with safety rules.
  • Utilizes electronic records to document and track all case management services and
  • referrals.
  • Complete necessary documentation to facilitate referrals to collaborative outside specialty
  • Other related duties as directed.
