Description:

Reporting to the Manager of Integrated Services, the incumbent will be responsible for the following functions:
- Providing client needs identification through ongoing assessment and client directed care planning
- Implementing client centered plan of care that focus on strengths, wellness and empowerment to support recovery and goal achievement
- Providing crisis prevention and intervention planning with the hope of preventing a relapse and/or hospitalization
- Acting as a resource person or an advocate to assist individuals in obtaining information/access to services/supports which are deemed needed
- Liaising with family/significant other and natural supports to facilitate the clients’ links within the community
Qualifications:
- Post-secondary degree in Social Work from an accredited university with current registration with the O.C.S.W.S.S.W; a Registered Nurse or Registered Practical Nurse with current registration with the C.NO. Bachelor’s Degree with a Social Service Worker Diploma and a current registration with the O.C.S.W.S.S.W; or Bachelor’s Degree with an applicable college diploma in Addictions and current registration with the C.A.C.C.F.
- Experience engaging and working with Indigenous Peoples in a community setting;
- Experience and a detailed knowledge of serious mental illness, usually gained by several years of working with this population;
- A demonstrated knowledge, awareness and sensitivity to Indigenous Cultures and Traditions; and
- Valid Ontario Driver’s Licence and access to a vehicle are required.
Hours of Work: 35 hours/week. Flexible hours are required to meet the position requirements.
Salary: Commensurate with experience and education within the range inclusive of HOOPP.
