Our programs and services are focused on assisting local Indigenous individuals and families in a real and practical way, from our Court Support programs that assist Indigenous offenders to navigate the judicial system, our Public Intoxication Program to our men’s and women’s yarning circles that connect people in the local community to the help and support they need. Five Bridges is a not-for-profit led by the local community for the local community.

KNOWLEDGE AND EXPERIENCE REQUIRED

  • Identify as Aboriginal and/or Torres Strait Islander
  • Minimum Certificate III in Community Services
  • First Aid Certificate
  • Mental Health and AOD qualifications desirable
  • Case Management and Mentoring experience
  • Experience working in Community
  • Lived experience
  • Excellent time management skills
  • Demonstrated ability to effectively perform duties in an organisation, achieving goals and meeting deadlines.
  • Sound knowledge of computer technology systems and programs to assist with managing tasks and developing reports.
  • Willingness to learn and follow directives.
  • Understanding of Mental Health and AOD
  • Drivers Licence and own reliable transport with appropriate insurances
  • Maintain Positive Notice Blue Card

Responsibilities:

ABOUT THE ROLE

The Case Worker / Mentor aims to provide support to Aboriginal and Torres Strait Islander participants that have been referred to the Five Bridges Public Intoxication - Case Management Program. Service provision requires an understanding of intergenerational trauma, substance abuse, health, homelessness and offending. Intensive case management can involve comprehensive needs assessment and goal planning, promoting holistic wellbeing and positive change.
Service users may be referred by the Murri Court or may be service users with no current court matters seeking support to make behavioural and lifestyle changes. Service users are complex and at risk, requiring individual support and case coordination to access, engage and maintain appropriate supports and services.

RESPONSIBILITIES AND ACCOUNTABILITIES

  • Focus on enhancing cultural connection and a sense of belonging amongst participants.
  • Case coordination, integration and engagement with other relevant support services and government agencies.
  • Mobile and centre-based case management.
  • Intensive case management with comprehensive needs assessment and goal planning, promoting holistic wellbeing and positive change for participants.
  • Develop and monitor individual treatment plans and maintain case management documentation, records, reports and data collection, utilising established case planning procedures.
  • Case conferencing with the Maroochydore MCC, EOM Project Support Officer and other service providers.
  • Provide alcohol education and presentations to participants.
  • Advocacy for service users.
  • Meeting KPI’s and deliverables as set within the service agreement.
  • Attend court sittings to aid participants with culturally appropriate supports.
  • Link participants with support and legal services.
  • Provide mentoring and guidance to participants within the program.
  • Effectively communicate Court stakeholders, staff, and other major key stakeholders, including DJAG staff and other CJG groups.
  • Facilitate improved links between Aboriginal and Torres Strait Islander communities and statutory workers, Queensland Police, Courts and other agencies and services that have interactions with the justice system.
  • Attendance at Community events and network meetings as appropriate.
  • Create connections and networks with local service providers across the Sunshine Coast and Gympie areas.
  • Other duties as requested by the CEO or Maroochydore Murri Court Coordinator.
