Description:

INFORMATION, INTAKE SERVICES

  • Respond to telephone calls, letters and e-mails from people seeking information about mental health services, how to access services and systems, and the mental health act.
  • Assess the service needs of the person seeking information and direct them to appropriate treatment and/or support services.
  • Provide a range of options for care and links to them: phone numbers, hours of service, locations, and waiting lists.
  • Assist callers by making telephone inquiries about services, eligibility, and what callers can expect.
  • Screen completed referral forms to determine whether the program applied for is suitable.
  • Request original consent forms to request hospital records.
  • Record referrals on database system and on log sheet.
  • Respond to follow-up calls from potential clients, family members, and referral sources.

EDUCATION

  • Post Secondary Degree/Diploma in Social Services or an equivalent

RELATED EXPERIENCE

  • A minimum of three (3) to five (5) years of experience in community mental health, with knowledge of community programs and resources in the Greater Toronto Area.
  • Experience providing service for people with diverse backgrounds.
  • Experience researching and helping people find appropriate information.
  • Experience with clinical records databases.

SKILLS REQUIRED

  • Demonstrated knowledge and ability to work with a Trauma Informed and Recovery oriented approach.
  • A thorough understanding of the resources and skills required to provide professional community information and referral.
  • Excellent assessment and communication skills.
  • Ability to work effectively as part of an interdisciplinary team.
  • Demonstrated knowledge of Mental Health, and relevant policies and legislation, such as the Mental Health Act and PHIPA is required
  • Demonstrated competency in building therapeutic alliance and in crisis intervention skills
  • Strong working knowledge of Microsoft Office applications and client service databases.

Responsibilities:

THE ROLE AND OPPORTUNITY

This is a blended role of Information, Intake and Referral Coordinator and Care Coordinator for Integrated Care Pathway for Schizophrenia (Partnership with Ontario Shores, CMHA Durham, and SHN). The clinician will have sound knowledge of clinical issues in the area of mental health and addiction as well as strong skills in complex care coordination between and within organizations. The details of each role is as outlined and as this is a new position there will be opportunities to enhance and fine tune the responsibilities:

  • The Information, Intake & Referral Coordinator serves as initial contact either by phone or in person; initiating basic clinical client screening; assessing requests made by the referral source and referring to appropriate resources; fielding incoming calls; developing and maintaining relationships with other community agencies to enhance available resources to clients; maintaining data base information.
  • As a member of this innovative new team, the Care Coordinator will work closely with partner organizations as we implement an integrated care pathway for individuals with schizophrenia. This role will support the establishment of partnerships across multiple organizations by streamlining the coordination across different services and care providers. The aim is to create a seamless and integrated experience for people with the diagnosis of schizophrenia that the organizations currently and collectively serve.
