Description:

An exciting opportunity has arisen within the Families & Therapies Division to be part of the ISCAN care co-ordination service, an integral part of the three ISCAN sectors, delivering a service to children and young people with disabilities and developmental difficulties. There is one vacancy of the Family Support Worker to be based in Caerphilly Children’s Centre but may be expected to travel to alternate sites.
We require applicants who have previous experience of working with this vulnerable group of children and families. Post holders hold and manage a delegated caseload and will need to be able to assess the needs of children and families, being be accountable for the provision of a co-ordinated range of services to meet need. Excellent written and verbal communication skills will be required as will empathy and the ability to deal with sensitive emotional issues effectively. All successful applicants must be able to evidence that they can co-ordinate meetings between relevant professionals across health, education and social care, develop effective relationships and work safely in a lone worker situation.
Previous experience of working within a health, social care or education setting is essential and applicants with relevant experience will be considered favourably. If successful you will be supported in the development of skills and knowledge in line with the role.
The post holder will be accountable for the provision of a co-ordinated range of services for children with disabilities and/or developmental difficulties, aged 0-18 years of age. The post holder will be an independent practitioner with the skills and ability to assess the needs of children and families liaising with the multidisciplinary team to secure appropriate provision. They will develop effective networks and maintain collaborative partnerships with relevant partner agencies such as education and social care.
The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
