Responsibilities:
The Joint Replacement Program Coordinator is responsible for the development, coordination, and maintenance of the care continuum for total joint replacement patients. This position provides leadership for all aspects of the continuum of care beginning before surgery through one year postop; including PSC, Perioperative services, inpatient units, post-acute care and all other applicable entities within the orthopedic service line related to evidence-based practice for orthopedic patients. The Joint Replacement Program Coordinator will plan, develop, and implement program goals and operations. This position coordinates and works with infection prevention and quality to continually improve care for the orthopedic population. The Joint Replacement Coordinator will develop, coordinate, and implement the clinical and business activities as well as the evaluation of patient outcomes through chart audits, database entries, and reports. This position will work collaboratively with physicians, post-acute care providers, families, and staff to develop and maintain a multidisciplinary Joint Replacement Program. Responsible for reaching total joint center of excellence and maintain the status.
