We are looking for aBand 7 Keyworker Senior Practitioner who can: Work flexibly within 37.5 hours per week, which might include some weekends, bank holidays and evenings as the pilot develops. Initially oversee and coordinate intensive support and intervention to a small number of CYP currently in hospital and their family/carers, this will extend to a larger at-risk group of children throughout the remainder of the pilot Work with existing health, social care, education and send services to coordinate packages of care for a small number of CYP and their family/carers with complex needs, either at risk of admission to hospital, or currently within hospital settings. Provide leadership and oversight of appropriate meetings including Care, Education and Treatment Reviews (CETR) ensuring resulting action plans are implemented and reviewed.
Contribute to relevant placement planning discussions with a key role in alerting systems if instability of placement occurs. Lead on ensuring CYP and their family/carers receive specialist advice and support from one trusted adult who will navigate and coordinate advice and treatment for other professionals involved with their care. Provide family focused support and challenge to system partners when necessary. Work within a multi-organisational landscape, working closely with systems to develop services able to deliver care packages to suit the individual needs of the CYP and their family/carers.
Ensure all CYP have a clear personalised care plan to promote their development, well-being and protect them from harm. Leading on coordination of support at pace to avoid admission to hospital Where hospital admission is necessary the postholder will ensure ongoing involvement throughout the inpatient period, facilitating timely discharge, well managed transition and full implementation of CETR recommendations for community support. Offer emotional and behavioural support and informed advice to CYP and their family/carers based on shared knowledge and experience of working with CYP and family/carers with complex needs. Attend and contribute to CETR and LEAP and tracker meetings ensuring recommendations/actions are completed in a timely manner.
Create and encourage a culture which prioritises keeping CYP within the home and local community by persuasively communicating with a range of teams and services. Ensure seamless, well-coordinated transitions between multiple systems. Advocate on behalf of CYP and their family/carers where required to ensure the necessary support is in place to enable them to remain within or discharged into the community and promote joined up, consistent care. Raise awareness/lead by example when educating the wider system on how to change the conversation so the focus is much more positive for CYP and their family/carers when speaking to other professionals.
Participate in system transformation encouraging strategic discussion and planning, identifying, discussing and resolving issues that inhibit excellence in the delivery care for CYP and family/carers. Provide supervision to junior staff within the teamsupporting team members in providing evidence-based care packages to suit the individual needs of CYP and their family/carers. Work across pathways and geographical areas as required, according to need and demand. Provide leadership in the development of high-quality services through the use of audit evidence-based interventions, goal-based, collaborative practice and Routine Outcome Measures.
Ensure ongoing participation of service users remains central and core to service developments throughout the duration of the project. Evidence ongoing and appreciation and learning from CYP and their family/carers stories and articulate this to inform service development and redesign
