Description:

The post holder is identified as a clinical expert and is required to have a high clinical profile ensuring the provision of effective and efficient care. The post holder is expected to work as a highly skilled practitioner, have expert knowledge of the Frail Older person and / or rehabilitation specialities and to continue to develop their practice and the service. Alongside a senior clinical role within the Nurse Practitioner service, the post holder will provide managerial oversight of the Advanced Nurse Practitioners, helping to set and deliver the vision for the service in conjunction with the medical leadership within the hospital.
The primary focus of the role is to work as an advanced, and often autonomous practitioner, providing patient centered clinical care in Neath Port Talbot hospital wards. This will include the assessment of patients in a range of inpatient, ambulatory and outpatient settings, acting as an expert decision maker. To independently assess patients who are either undergoing active rehabilitation or the frail older person of all types at any stage of their management pathway. To devise management plans for ongoing care, breaking bad news, provide feedback of results to the patient while supporting both patient and their family/carers. To use clinical skills such as history taking, physical assessment and non-medical prescribing within clinical competence.
The post holder will be expected to exercise a high degree of professional autonomy and make critical judgments to meet the expectations and demands of the role. The post holder will be expected to work at an advanced clinical level within their level of competence and to work flexibly as part of a wider team.
Alongside the clinical role the post holder will act as the Lead Nurse Practitioner, working alongside the medical leads for rehabilitation or elderly care, to deliver a safe, effective and forward looking service. The role will include ensuring effective professional leadership to Advanced Nurse Practitioners in order to secure and assure the highest standards of clinical care; ensuring the availability of the appropriate support services; ensuring a quality service and maintaining a high standard of infection control; securing and assuring the highest standards of clinical care; ensuring the availability of the appropriate support services; a quality service and maintaining a high standard of infection control; and providing an accessible and supportive presence to whom patients and their families and staff can turn to for assistance, advice, support and early resolution of concerns.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
