Under the direction of and in collaboration with a Clinical Manager, the Lead Care Manager, RN supports Care Management through development and project management of care management workflows, educating others on care management processes, and implements best practices across various sites for optimal patient care delivery. Responsibilities also include assisting both internal and external customers in providing patient care within the scope of practice defined by the California Board of Registered Nursing. The Lead Care Manager RN is a subject matter expert in care management resources and appropriately triages patients to the right level of care at the right time for cost effective, patient-centric care.

REQUIRED QUALIFICATIONS:

• Graduate of an Accredited School of Nursing
• California Registered Nurse License upon hire
• 3 years of CM experience or equivalent experience coordinating the care of patients
• EMR experience

PREFERRED QUALIFICATIONS:

• CCM or MCG Certification
• Experience in a leadership role
• EPIC, IDX, Allscripts, Meditech, Cerner experience
