Description:

As required, to provide a ward based clinical pharmacy service. Promote the safe, rational and cost effective use of medicines in accordance with the Medicines Management Group recommendations, Medicines Efficiency project and other evidence based guidelines. To evaluate the clinical appropriateness, safety and legality of prescriptions based upon the patients diseases, co-morbidities and other medication and treatments taken, contacting the prescriber to make agreed changes as appropriate. Drug history taking and medicines reconciliation at admission and discharge Promote efficient discharge planning inclusive of patient counselling and education.
Follow procedures to reduce medicine waste (e.g. by using patient own drugs) Ensure efficient supply systems (e.g. one stop dispensing), reducing the risks of critical missed and delayed doses by working with the technicians/ATOs and dispensaries. Work with the ward nurses, dispensary and pharmacy ward team to improve patient flow.
Attendance on consultant wards rounds, as appropriate. Answer or refer medicine information queries as appropriat
