Description:

The successful candidate will have proven experience within a primary care setting and active knowledge, experience and interest in diabetes management. Education Role Work alongside our well being co-ordinator to deliver education to our patients in the self-management of their diabetes. Empower patients and encourage uptake in diabetes education courses Maintain links and communication with the patients, carers and relatives around condition management. Clinical Role To facilitate specifically designed and designated clinics based at Park Lane Practice and Phoenix Surgery.
To demonstrate advanced knowledge of diabetes and its co-morbidities using appropriate skills and innovative practice to ensure the delivery of equitable, quality, safe care that is up to date, evidence based and within local and national guidelines. To manage a caseload of adult patients with diabetes, both Type 1 and Type 2 to deliver holistic, individualised care. To support, empower and enable patients to make informed decisions regarding their own diabetes care, as far as possible, by engaging with the patient, their family and carers and establishing good relationships to ensure patient centred care. To undertake diabetes clinics within the practice, reviewing the care and management of patients with diabetes, interpreting laboratory results and the patients own self- monitoring results to ensure accurate diagnosis, effective treatment and longer term management .
To make decisions regarding the diagnosis, initial and on-going treatment, management and prescribing for people with diabetes. To screen, prevent and detect early complications of diabetes and its co-morbidities, whether physical, psychological or social. To maintain links with the diabetes specialist service in secondary care, referring patients to specialist clinics as appropriate and according to local guidelines. To liaise with other health and social care professionals or voluntary organisations, where appropriate, both hospital and community based.
To be able to make decisions regarding the most appropriate medication or insulin regimens for the person with diabetes, taking account of their home, social and occupational needs, their physical and psychological needs and ability to self-manage. To work autonomously as an independent practitioner, demonstrating expertise in the assessment, prescription implementation, planning and evaluation of patient care. QOF work Work alonside as a lead of a multi-disciplinary team to actively plan, manage and undertake QOF work within the practices
