Description:

We have a part time Band 6 vacancy (30 hours) for a learning disabilities nurse to work in our small clinics team, part of the wider learning disabilities health MDT. If you are an enthusiastic and motivated learning disabilities nurse, keen to work as part of an MDT, we would love to hear from you. This post will be working with both young people aged 14 - 18 with learning disabilities or who are autistic as well as adults with learning disabilities. You will be working closely with all the clinic team (and the wider MDT) supporting people to have physical health checks and liaising with both primary and secondary care services to ensure reasonable adjustments are in place so people can access the appropriate health support. You will also be working closely with dental colleagues, developing pathways for young people with learning disabilities or who are autistic aged 14-18 needing dental admissions in the local hospitals - this will include leading on the development of person centered care plans and hospital support plans with the young person and their families so they can have successful dental input.
The post holder will work collaboratively with people with learning disabilities, their carers, professionals and agencies to provide a high quality, comprehensive and dynamic specialist nursing services with the central aim of maintaining and improving the health and wellbeing of young people & adults with learning disabilities. This will include but is not limited to, working directly with service users who have complex healthcare needs, care coordination, supporting others to act as Health Facilitators, supporting health providers to make reasonable adjustments to care pathways and engaging in a range of health promotion activities.
There is a lot of variety in the workload, you will work with a number of different health providers and you will need to be able to work independently. It is a busy but very friendly and supportive team with lots of opportunities to develop and learn new skills. We would be willing to consider a development role for Band 5’s who may not feel they have the skills yet for a band 6 but are keen to develop these.
