Description:
In conjunction with the other post holders present, to contribute to the post-take review of the overnight new admissions. During the rest of the day the post-holder will be expected to help oversee the running of the unit, to supervise the juniors and support the multidisciplinary team. At 11:30am there is a multidisciplinary board round where all patients are handed over and discussed with nursing and therapy teams. Ongoing review of the AAU short stay patients, communication with families and to oversee a smaller board round at 15:00pm to ensure all tasks identified on the morning round have been completed.
To provide continuity to the patients and staff on the AAU through twice daily review of short stay patients. To identify patients suitable for ambulatory care (SDEC) and to support the nursing staff on the SDEC in their roles. To provide phone support to GP enquiries. When on SDEC clinic, to review patients within the clinic and provide senior clinical advice.
To act as a role model for acute medicine, high quality medical care and knowledge and to contribute to the supervision and education of the junior and multidisciplinary team. Whilst on call to be proactive in facilitating the movement of patients through the emergency care pathways by interaction with the ED and ambulatory units to aid patient flow and optimise patient care and experience. Help to develop innovative and high-quality care pathways and protocols that will improve the experience and the quality of their care delivered to our patients. Contribute to the service audits and governance of the pathways and of the unit.
Within the post there will be a requirement for leave cover within the ACP group. A commitment to take part in CPD activities on a regular basis To participate in the general physician on call weekend rota 1:13. Each weekend has 2 consultants rostered. The weekend works provides a review of all the patients on the AAU as well as seeing all the new patients on the take as well as providing support to the general medical wards and junior staff
