PACE Baton Rouge (Program of All-Inclusive Care for the Elderly) is a nonprofit health and supportive services program designed to assist seniors who want an alternative to long-term care placement. PACE is sponsored by the Franciscan Missionaries of Our Lady Health System (FMOLHS). The mission of PACE Baton Rouge is to enable the aging population to live with dignity in their communities. With PACE’s assistance, participants can remain in the secure surroundings of their own homes and communities while providing family members and caregivers with much needed professional guidance; physical relief and emotional support.
Our team works together to develop individual care plans, goals and carefully monitors progress. Our multi-skilled team-consisting of a doctor, registered nurse, social worker, physical therapist, occupational therapist and registered dietitian-works together to develop patient goals, plan care and carefully monitor progress.

JOB SUMMARY

The Manager of Quality is responsible for the implementation and management of the performance improvement program for the PACE organization. Serves as a resource to leadership, Instrumental in all process improvement measurement and data analysis. Ensures compliance with Louisiana Department of Health and CMS regulatory agencies. Instrumental in Improving Organization Performance and takes the lead in all quality measures for all the PACE sites. Responsible for oversight and training of all Quality Coordinators. Works Collaboratively with clinical and medical teams. Oversight of regulatory readiness, reporting and works with leaders to achieve organizational performance goals. MINIMUM REQUIREMENTS

  • Experience -3 years experience in Quality Prior experience leading quality and performance improvement initiatives or in project management Proven track record in leading system change
  • Education - Bachelor’s Degree

  • Strategic Leadership and Operations
  • Responsible for the leading the Quality Committee work group and the participant safety and quality transformation pillar team. On other administrative committees and attends meetings to provide consultation and recommendations regarding the division’s area of expertise.
  • Responsible for coordination and oversight of quality and performance improvement initiatives within PACE that directs and leads the change to positive outcomes and improved quality performance. Responsible for yearly assessment and update of the PACE Quality Plan.
  • Responsible for the oversight of all Quality Coordinators and development and implementation of program wide and center specific quality indicators, holding each PACE Center accountable to quality initiatives and processes are in place to monitor, direct and measure all activities related to quality improvement, updating quality metrics and identifying opportunities for improvement.
  • Coordinates with National Initiatives in PACE Quality Management including developing of corrective action plans for CMS reports, creating timelines and deliverables for workflow development, implementation and monitoring for selected quality measures.
  • Responsible for the Monitoring of the Environmental Rounds and ensuring safety in the PACE Centers.
  • Quality and Performance Improvement
  • Responsible for maintaining survey readiness related to CMS and Louisiana Dept. of Heath.
  • Recommends policies and procedures that are consistent with the mission and the operating goals and objectives of Franciscan PACE Inc. Assists senior management in the development and implementation of programs, policies and procedures to enhance the performance improvement and medical staff strategic directives. Recommends updates to Interdisciplinary Team, rules and regulations, and core privileges when appropriate.
  • Identifies performance improvement and participant outcome indicators. Research literature for clinical practice guidelines and national and regional benchmarking for comparisons. Responsible for planning, designing, implementation and maintaining system dashboards in support of operating mechanisms to address deficiencies and performs data entry analysis of all related performance improvement activities, to compile data and analyze data for significant trends or occurrences.
  • Promotes and encourages the growth and development of staff members by encouraging their participation in continuing education activities such as professional conferences, seminars, and workshops.
  • Strives to promote the quality and efficiency of his/her own performance by remaining current with the latest trends in field of expertise through participation in the job-relevant seminars and workshops, attendance at professional conferences.
  • Enters Data needed on a quarterly basis in HPMS, required by CMS. Assures the Data PACE 3 information is uploaded to the necessary entities. Coordinates the necessary entries for the HOS Survey on a yearly basis and works with all agencies necessary to improve the outcomes of PACE.
  • Provides presentations to the Franciscan PACE Board of Directors and assists other quality coordinators on data presentation.
  • Coordinates Infection control program with quality assessment performance improvement.
  • Coordinates the Emergency Preparedness Disaster Planning, working with the community and implementing the Federal mandated guidelines.
  • Collaboration and Partnership
  • Provides ongoing expertise and support for the organizations performance improvement plan and oversees and coordinates the development and implementation of program wide and center specific quality indicators giving operational support for the organizations approach to performance excellence.
  • Develops positive relationships between the community, physicians and professional affiliations by acting as the PACE Quality Representative. Promotes and maintains relationships and communications within the division and other areas.
  • Identifies and monitors quality-related accreditation requirements and ensures compliance. Tracks performance relative to national benchmarks providing feedback/data to leadership and guidance toward effective performance improvement interventions.
