Description:

Maternal Early Childhood Sustained Home-Visiting (MECSH) is a licenced intervention programme delivered within Tower Hamlets GPCG 0-19 Service. The MECSH Specialist Practitioner will work with the Clinical Lead MECSH portfolio holder and Health Visiting teams. They will have the key responsibility to support the Health Visitors to effectively implement the MECSH Programme within their practice and work collaboratively with them to improve the equity and quality of child and family health service delivery. They will support clinical practices for MESCH within the service through training, reflection, supervision, and case review and have a caseload of MECSH clients.
They will be responsible for the allocation of MECSH clients within the teams and act as a role model for practice. They will provide MECSH training for new staff. In partnership with key stakeholders: Increase the boroughs MECSH offer by reviewing, developing, and implementing operational pathways to identify, recruit and deliver MECSH to families in Tower Hamlets in line with the fidelity of the program. Support coordination and delivery of MECSH training for Health Visitors and MECSH Informed sessions for other professionals within the Health, Social Care and Early Years sector.
Deliver MECSH Supervision groups to foster a positive and collaborative team culture, promoting professional development and job satisfaction. Implement mechanisms for ongoing program evaluation and quality assurance, collecting and analysing data to measure outcomes and impact. Develop a strong partnership with relevant leads from MECSH UK and MECSH Australia contributing to successful program delivery in Tower Hamlets. Contribute to the development of the Tower Hamlets Family Hubs and Best Start in Life offer for related workstreams, such as Perinatal Mental Health.
Main duties and responsibilities CLINICAL DUTIES To hold a small caseload of MECSH clients Undertake home visits in accordance with MECSH Use high level interpersonal and communication skills to provide and receive complex and often highly sensitive information. Use strengths based, solution focused strategies and motivational interviewing skills to enable families to develop behaviour change strategies. Use professional and clinical judgement to undertake detailed assessments of clients and their families and analyse complex family situations to develop appropriate clinical interventions. Actively engage and work with all members of the extended family to achieve positive outcomes for children.
Apply knowledge and skills to a range of clinical and family situations Work effectively in potentially highly distressing or highly emotional circumstances, e.g. safeguarding children or family breakdown. At all times keep the safety of the child and family in mind, and work within local safeguarding policies as appropriate. Contribute to the Healthy Child Programme, including professional assessments of childrens growth, development and language skills.
Complete full, accurate and contemporaneous records MANAGEMENT AND LEADERSHIP Support the delivery of MECSH Programme components and training to Health Visitors. Actively contributes to the development of sound clinical practice in the delivery of the MECSH Programme. Acts as a resource and mentor to others in relation to MECSH clinical practice Provides effective and timely support to Health Visitors delivering the MECSH Programme in Tower Hamlets in accordance with licence and program requirements. To include provision of accurate and relevant advice and exercising of significant judgement in appropriate interpretation of programme and licence requirements in response to site needs.
Provide monitoring through reflective supervision to Health Visitors to ensure that expertise in clinical methods is developed and maintained, that the service offered is of high quality and the programme is implemented with fidelity. To facilitate MECSH community groups. Development of MECSH documentation and materials in response to site needs. Contribute to and attend the ongoing MECSH Community of Practice Brief and yearly conference.
PROFESSIONALISM For the MECSH leads and Health Visitors to use the MECSH model to initiate, plan and co-ordinate clients care in consultation and collaboration with other professionals and agencies involved in providing services to women and their families. This includes initiating Early Help Assessments (EHA) and taking lead professional role in safeguarding situations where required. To be involved with the Clinical Lead MECSH portfolio holder in analysing reports on data for service development and develop quality improvement measures to assure data quality, completeness, and accuracy as well as improvements in programme delivery. Develop systems for user involvement and support clients to offer feedback on the service received, integrating this into local or national improvement measures.
Be aware of, inform and abide by provider services approved policies, standards and quality assurance initiatives. TEACHING AND TRAINING Attend train the trainer programme to be able to provide training on MECSH Effective provision, identification, and development of MECSH training programmes with participant feedback. CLINICAL GOVERNANCE Maintain professional registration and revalidation in line with NMC requirements. Participate in individual management/clinical/safeguarding supervision support and facilitate the training and development of team members who need to improve service delivery.
This includes record keeping and improving clinical competency. Through mentorship and preceptorship Maintain contemporaneous records in line with NMC guidelines and employer policies. Work within the clinical governance framework to ensure that governance is embedded in clinical practice. Monitor the quality of care delivery and record keeping ensuring wherever possible that it is evidence based.
Report any concerns regarding professional practice to the Locality Clinical Manager, Clinical Lead, or an appropriate other. Participate in complaint / incident investigation, problem solving and remedial action where necessary. Work within guidelines to identify and manage risk utilising expertise within or outside the organisation when needed. Ensure compliance with health and safety requirements with particular attention to lone worker and manual handling policies.
RESEARCH Detailed and comprehensive documentation of MECSH implementation support processes is prepared in a timely manner to support research. To take part in audits, service reviews and research. They will work collaboratively with the Health Visiting Locality Clinical Managers
