Job description
The Medical Coder / Biller assigns diagnostic and procedure codes to patient medical records, enters patient information into the Electronic Health Record and Billing Software, submits claims, and follows through till timely payments are received. Generally performs coding on all types of cases and services. May be assigned to either office, outpatient or inpatient settings. Utilizes current and appropriate rules and guidelines of code sets such as ICD-10, CPT and HCPCS.

Responsibilities:

  • Analyzes medical records to identify significant elements and represent them as specific diagnostic and procedural codes
  • Collects health information as documented by physicians, qualified health care professionals or medical specialists and code them appropriately
  • Reviews appropriate provider documentation to determine principal diagnosis, co-morbidities and complications, secondary conditions, and surgical procedures.
  • Accurately selects codes from the International Classification of Diseases (ICD), American Medical Association Current Procedural Terminology (CPT) coding system, and Healthcare Common Procedure Coding System (HCPCS).
  • Analyze health records and follows official coding rules and guidelines to review and.
  • Consults medical specialists for further clarification and understanding of items on patient charts to avoid any misinterpretations
  • Enters codes into the Electronic Health Record and Billing Software,.
  • Reports to their supervisor their productivity status and daily issues.
  • Identifies and reports areas of concern with respect to improper coding and documentation.
  • Maintains confidentiality of patient records at all times.
  • Submits claims to insurance companies, claim administrators or third party payers
  • Identifies coordination of benefits, and bills secondary or tertiary insurances
  • Follows up payments from payers and patients
  • Evaluates and timely appeals denied claims
  • Submits reports relative to work performed
  • Maintains good client relationship in the course of duty
  • Collects, analyzes and presents solutions to coding and billing related information and issues
  • Performs other functions as assigned.
