Description:

JOB SUMMARY

As a member of the medical team you will be responsible for contributing to the development of the community and in-patient services.
The postholder will work organisation wide rotating between the community service and inpatient service to drive forward multidisciplinary working with internal and external professionals to support continuous improvement in patient care. There is a particular focus on supporting care in the persons own home using face to face and virtual systems.
The postholder will be involved in education, quality improvement and research.
The person in this role will have excellent communication skills with the ability to communicate confidently and effectively at all levels within the Ayrshire Hospice.

Responsibilities:

Reporting to the lead consultant, the post holder is responsible for providing direct clinical care and advice for patients with specialist palliative care needs both within working hours and as agreed out of hours.

The postholder will:

  • work directly with the community consultant and community team to provide specialist palliative care advice to patients, their carers and healthcare professionals for individuals being cared for in their own home or other homely setting.
  • Support care of patients in a community hospital setting
  • Support complex discussions regarding medical care and future care needs
  • Work directly with the multidisciplinary team to support patients care within the hospice in-patient unit with a particular focus on development of management plans, communication with external healthcare professionals
  • Support quality improvement across all hospice settings
  • Support research across all hospice settings
  • Support education of professionals in health and social care internally as well as in primary and secondary care
