Description:

This exciting post will be working in the Memory Assessment Service (MAS) providing assessment, potential diagnosis and signposting to other services and support networks for individuals aged 65 and over referred to the service with cognitive impairment.
We are looking to recruit a Band 6 Memory Assessment Nurse.
The posts will work in the areas with the highest waiting times and will work with colleagues to reduce the waiting times in that area. There will be a requirement to travel between sites throughout Nottinghamshire.
To work as part of the multi disciplinary team to screen for appropriate referrals
To complete an assessment of clinical presentation and symptoms
To share information amongst the multi disciplinary team and create appropriate and person centered care plans
To refer for appropriate diagnostic procedures eg scans, neuropsychology, occupational therapy.
To make independent diagnostic decisions based on consultation, history, presentation, pathology and/ or imaging results and to seek supervision from service medical support in complex diagnostic situations.
To complete accurate records

This role is working within Nottinghamshire Healthcare NHS Foundation Trust within the Mental Health Services for Older People directorate.
The successful candidate will work within the MAS service and will review/accept referrals for individuals over the age of 65 with cognitive impairment, completing assessments for the same to ascertain whether or not a Dementia diagnosis is appropriate.
To screen referrals for appropriateness
To undertake initial assessments and create a treatment plan
To liaise with the MDT to explore symptoms and causes
To refer for additional assessments
To signpost for post diagnostic suppor
