Description:

We are looking for an enthusiastic Community Psychiatric Nurse/Mental Health Practitioner to work in our Older Adults Service. With an interest and passion for caring for patients with mental health needs including dementia. Previous experience is preferable however we welcome newly qualified applicants with a keen interest to learn and develop their skills.
You will work in our Older Adults Home Treatment Team providing mental health care and treatment using appropriate pathways including the Dementia Living well Strategy in line with NICE guidelines.
A good understanding of Mental Health Framework and legislation in addition to Clinical Risk policies and procedures is desirable.

The role is full-time (37.5 hours per week) however interested part-time applicants will be considered.
The service operates 7 days per week, so flexibility with regard to unsocial hours/ shift patterns will be required.
Participating as a member of a multi-disciplinary team managing a caseload of patients
To undertake assessments of patients, involving their carers and family members in the patient’s care and care plans.
To be responsible for the formulation, execution, and evaluation of care plans for a designated number of patients who may present with significant complexity, in consultation with the patient and the multi-disciplinary team.
The post holder will direct and supervise Support Workers and/or students.
Post holder will provide advice to other members of the service and agencies on the needs of service users.

Responsibilities:

Have the relevant professional qualification for a Mental Health Nurse or equivalent Professional registration required.
Interest in working with older people with mental health problems
A passion and understanding for working with Older People
Good understanding of the relevant legislation surrounding the Mental Capacity Act / Mental Health Act
The ability to assess risk in both routine and urgent situations
Be a car driver with the ability and readiness to travel
