Coastal and North West Sussex Children Adolescent Mental Health Service (CAMHS) have recently received funding to develop and expand the fantastic services we offer to young people in our community. We work with a range of children and young people who present with a multitude of mental health problems.
We are looking for a skilled mental health practitioner who will take the lead in working with our adult colleagues to identify those young people who will require ongoing care in post 18 services and to support their transitions. We have an identified lead for transitions and an integrative policy framework to guide our practice.
The post will suit a practitioner from either child or adult services who has a commitment to ensuring the achievement of good quality transition plans.
Working across West Sussex and in collaboration with CAMHS and AMHS you will play a key role in ensuring our young people will receive a timely and smooth transition from their CAMHS team to their local Adult Mental Health Team. As part of this role you will be involved in working with the family and the MDT to establish the right level of future care needs for each young person. This could involve working with our colleagues in the ADMHS or Tier 2 level services such as Sussex Talking Therapies, Pathfinder and signposting to other local support services.

The post holders will receive clinical and line management supervision and support as well opportunities for in-house training. Some post-registration training would be very welcome, however the Trust is keen to develop its Mental Health Practitioners and there are lots of opportunities for CPD and being part of the wider Sussex CAMHS team through Away Days and other specialist training opportunities.
This post offers the Trust’s £4,000 Incentive for band 5-7 Nurses & Practitioners, as well as a relocation package up to a maximum of £8,000 (subject to terms and conditions).
In addition to the part of role outlined above, around transitions work. As the role progresses and we begin to fine tune the transition process, their maybe other opportunities with West Sussex CAMHS to gain further experience in assessment and treatment of young people.
This would include undertaking initial assessments for children and young people referred into the service, ensuring safeguarding, risk management and care planning are implemented to the highest standard, and at all times that the service offers effective patient-centred care. Holding a diverse case load of young people with complex emotional needs which requires significant attention to detail particularly around risk management in the community, thus co-ordinating and working with external agencies such as social care and education etc. You will have the opportunity to co-facilitate groups and treatment alongside multidisciplinary team colleagues to offer evidenced based care.
The post entails working in a lively, busy multidisciplinary CAMHS Team delivering high quality interventions to children, young people and their families, including assessments, individual and group work. You will get the opportunity to work with colleagues of all disciplines.
