Description:

  • Position will be based in the Needs Assessment Service Coordination Team on a fixed term contract or secondment (approx. 12 months)
  • This will be a unique and specialised role within the team reporting directly to the CTL; working closely with the CTL and Project Manager.
  • An exciting opportunity for a mental health professional who is keen to be involved in an important project focusing on enhancing whaiora recovery and system change.
  • You will be part of a project team, involving Te Toka Tumai managers and leaders, NGO providers, whaiora and their whanau, and consumer advisors.
  • Full-time (40hrs) position working Monday to Friday, based in a central location in Epsom right next to Cornwall Park

Exciting one year secondment opportunity available for the Housing and Recovery Change Project.
Housing and Recovery services are a range of residential support services purchased by Te Whatu Ora within each district. The service aims to provide those who have high and complex mental health needs with intensive support to achieve personal recovery and functional rehabilitation; ultimately individuals are supported to move towards the life they want to live, in a community of their choice.
Te Toka Tumai (formerly ADHB) purchases 116 beds of Housing Recovery services from seven providers; this project is underpinned by a change proposal with the aim to scope and re-configure existing services that may better meet the needs of whaiora who require long term supportive housing; at the same time improve in service delivery and recovery outcomes for whaiora who are receiving services.
You will be working closely with whaiora, their whanau, and NGO providers to review and develop support plans which drawn on proven models of rehabilitation, address inequitable health outcomes, and are part of a wider significant progressive change initiative.
We are seeking an experienced mental health professional who is passionate about improving service delivery and who is also a champion for personal recovery, social inclusion, and whaiora centred services.
You will recognise everyone has potential to grow, and Whaiora are supported to increase autonomy and participation in valued life roles by developing skills and resources that they need.
If successful, you will be based with the mental health Needs Assessment Service Coordination (NASC) team located at Greenlane Clinical Centre, though your role will be more specialised and you will primarily be tasked with work relating to the Housing and Recovery re-design project.
As this is a community based role, a full and clean driver’s license is required.

NGA PŪKENGA MOTUHAKE | IMPORTANT SKILLS

As this is a unique opportunity with focus on project work; the successful candidate will need to be skilled in clinical assessment and biopsychosocial formulation.
The ability to translate assessments and formulations to detailed support needs and recommendations will be required.
This role will be ideal for a mental health professional who has experience or passion in mental health rehabilitation, and improving social outcomes for whaiora with complex needs. A person with particular interest in evidence based and proven models of service delivery for mental health recovery and rehabilitation is desired.
You will need to be flexible, self-directed, and have excellent interpersonal skills.
You will carry out comprehensive support needs assessments that are objective and without bias, whaiora centred, and provide recommendations for service improvement based on your assessment
You will be skilled at report writing, clinical documentation, and have excellent time management skills.
You will work closely with the Clinical Team Leader and other key stakeholders.
