Description:

The integrated care practitioner role is well established within Bracknell. If you are energetic, a team player with a good skillset for autonomous working and decision making; you will be the right person to come and work as part of the Integrated Care Team.
The role is truly situated in between the services to prevent people falling between the gaps; you will be able to guide the person on correct pathway so there are “no wrong doors”. You are working closely with community nursing colleagues as well as local authority colleagues from Bracknell Forest, discussing new referrals and completing home visits.
This is a hybrid role, whilst being able to work from home, you’ll also have opportunity to work at Bracknell CMHT OA, Church Hill House and Bracknell Forest Council, Time Square.
Y ou will be a well-respected member of Bracknell CMHT for Older Adults and the Integrated Care Team, this is a matrix management model - your operational supervision will be provided by the service manager of the Integrated Service, your clinical supervision will be provided from Bracknell CMHTOA.
Your caseload will come from the work you undertake within the Integrated Care Team, you will work closely both Adult Social Care and CMHTOA in Bracknell, you’ll attend referrals meeting to provide steer as part of the integrated work.
To represent BHFT mental health services at locality LAP/Cluster/Care Home meetings. To screen referrals, identify any further information which needs more exploration to be able to identify a correct pathway.
To provide clinical leadership, and offer mental health expertise & guidance to professional colleagues and assist in formulation to support integrated decision making and care provision
To meet the multiple and often complex needs of frail patients with long term conditions who are discussed, through on-going joint assessment of health and wellbeing, including risk assessment, developing, implementing and evaluating intervention plans.
To support the multidisciplinary team to identify which patients discussed could benefit from mental health intervention.
To work as an autonomous practitioner, taking responsibility for mental health assessments establishing a formulation and implementing the clinical management required.
For those patients not requiring secondary mental health input, to support navigation of the health and social care system by recommending the most appropriate level of psychological support from the range of provisions available.
To hold a small caseload requiring more complex mental health support.

Responsibilities:

Degree in Mental Health Nursing or equivalent professional qualification or Dual Qualification eg RGN
Experience of clinically supervising junior and unqualified staff and mentoring students.
Experience of working effectively in a multi-disciplinary team and multi-agency environments.
Exceptional interpersonal, influencing, communication, observation and reporting skills. Ability to engage and communicate with a wide range of professionals, staff and other service stakeholders
