Description:
To offer a comprehensive therapy service to the resident population of Mid Nottinghamshire, providing a multi-disciplinary and multi-agency approach to maximise patient function, well-being and self-care planning. To provide high quality holistic evidence based care to patients / service users that is responsive, effective and efficient. To facilitate and support timely discharge and prevent inappropriate admissions by supporting patients/ service users to remain as independent as possible and maintain or improve their level of function and quality of life. The role will have visible clinical and management leadership responsibilities ensuring delivery of the teams in achieving all QIPP and performance targets, including uptake of clinical supervision, mandatory training, PADs and undertaking clinical audits.
This post is the first line of contact for GPs and Primary Care ensuring positive relationships are developed and concerns dealt with immediately. The post will facilitate effective, positive partnerships with all Health and Social Care, CCGs, Secondary Care and key stakeholders and will develop and progress the teams, breaking down barriers by attending Multidisciplinary team meetings, ensuring patients are signposted to the most appropriate service to meet their needs. The post holder will lead the vision for embedding the five fundamental values of trust, honesty, respect, compassion and teamwork through out all care giving settings within the locality
