REQUIREMENTS

Licensure / Certification / Registration:

• BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American HeartAssociation or American Red Cross accepted.
• Licensed Practical/Vocational Nurse credentialed from the Florida Board of Nursing obtained prior tohire date or job transfer date required.

Education:

• High School diploma equivalency with 2 years of cumulative experience OR Associate’sdegree/Bachelor’s degree OR 4 years of applicable cumulative job specific experience required.

Assist with processing and transmitting data of the Minimum Data Set (MDS) Assessment and Care Planning to required authorities.

• Develop resident care plans based on the comprehensive MDS assessment.
• Ensure that all regulations are met regarding the MDS assessment process and obtain appropriate signature for all assessments.
• Coordinate discharge dates and participate in the discharge planning process.
• Maintain MDS computer system and equipment.
