Description:

The post holder will be required to assist in the delivery of a comprehensive administrative service to support management and clinical staff within the Digital Care Hub. The post holder will undertake the processing of patient referrals along with assisting with setting agenda’s and taking minutes of meetings. The post holder will also make and answer telephone enquiries, assist in the co-ordination of caseloads, using System One and assist with the promotion and development of the MyCare24 portfolio of services.

In pursuing these duties the post holder will ensure compliance with the NHS Constitution and be familiar with the NHS England Guidance ‘Understanding the new NHS’.

Fixed Term 4 months 29 hours per week
The post holder will work under the supervision of the Digital Care Hub administrative team and will undertake delegated duties, for which they have received training and at which they are deemed to be competent. The post holder will perform tasks independently and under indirect supervision as set out standard operating procedures (SOPs).
The post holder will adhere to all Trust and Directorate policies and procedures and will act in a manner to ensure the confidence of the general public at all times. They will ensure that adequate precautions are taken to safeguard themselves, patients, staff and property of the Trust.

KEY PERFORMANCE AREAS

Administration
Register patients on to SystmOne and other clinical systems.
Maintain patient records, ensuring accuracy of clinical systems.
Plan tasks in line with policies, guidelines, standard operating procedures and protocols.
Coordinate caseloads, ensuring appropriate scheduling of patient contacts.
Receiving and processing referrals.
Assisting with the development of marketing materials.
Actioning tasks allocated by clinical team.
Demonstrating duties to new starters from time to time when needed.
Report adverse incidents and near misses through the AEF system and bring to the attention of the appropriate staff in the hub.
Providing other administrative services as required eg, photocopying, sending mail, minuting meetings.
Be responsible & accountable for own behaviour and know when to seek support and advice.
Support patient centred care.
Escalate concerns where appropriate.
Communication
Communicate appropriately and sensitively in different settings, using a range of methods/styles, enhanced by underpinning knowledge and experience
Act professionally and appropriately in situations, understanding that there may be boundaries to confidentiality, seeking support and advice.
Ensure all data related to patient care is entered onto the relevant systems, maintaining confidentiality, e.g. patient care records, SystmOne.
Monitoring generic email inbox prioritising, acting on and directing mail appropriately.
Liaising with internal and external partners to support the delivery of patient care.
Receiving and recording feedback.
Taking calls from patients to onboard them onto the caseload.
Education and Development
Maintain own competencies and educational development.
Attend the mandatory updates as per Trust policy.
