Description:

The post holder will be a qualified neonatal nurse (QIS) competent to provide a high standard of professional practice within the Neonatal Community Outreach Team.
You will participate in the assessment, planning, implementation and evaluation of babies and their families following discharge from the Neonatal Unit at Southmead Hospital who meet the referral criteria to the service, including those with complex neonatal conditions. The postholder will actively support the Neonatal Community Outreach Lead, assisting in the safe, effective, and efficient management of the Outreach team caseload and deputising in their absence.
You will be part of an MDT working from a level 3 Neonatal Intensive Care Unit within the Southwest Neonatal Network serving the northern part of the network. The unit has 34 cots comprising of 8 intensive care, 8 high dependency and 18 special care cots. The team are committed to providing quality care; highly motivated to learn and be part of a dedicated team with a strong focus on family integrated care which extends the care to the home environment.
You will assume responsibility for undertaking tasks delegated by the Outreach Lead including mentoring roles, auditing, and clinical governance, and will promote an environment that is conducive to learning, supporting, and participating in the supervision/teaching of new staff, students on placement and other members of the multidisciplinary team.

Independently participate in the assessment, planning, implementation and evaluation of babies and their families following discharge from the Neonatal unit.

  • Work with other members of the outreach team to ensure a high quality of care is maintained.
  • Demonstrate excellent communication and leadership skills.
  • Act as a resource of specialist knowledge and expertise to ensure optimum and appropriate care delivery to babies and their families in the community and when discharge planning.
  • Work alongside parents and caregivers in the assessment of needs and how they may be met within the home including providing instruction, demonstration and written information in Basic Life Support and Safe sleeping in accordance with recognised body once relevant training is obtained.
  • Participate in the identification and delivery of Palivizumab immunisation clinic during the specified period once necessary training is obtained.
  • Lead and support staff and student nurses in the absence of more senior nursing grades.
  • Participate in teaching, supervision and mentorship of students and other staff.
  • Act as team leader for advice, expertise, and support in the absence of senior nursing staff.
  • Demonstrate effective time management and leadership skills.
  • Work closely with Outreach Lead to further develop managerial and leadership skills.
  • Demonstrate continuous responsibility for the management of the caseload as required.
