Description:

To use specialist Neonatal nursing skills to promote an integrated nursing service to sick infants and their families between hospital and community and enable infants to be cared for at home whenever possible. To act as a resource for both primary health care and secondary health care where specialist Neonatal expertise is required.
An exciting opportunity has risen to join the Neonatal Outreach (Community) Team. The team covers East Berkshire, Buckinghamshire and some border areas and has a varied workload with complex patients at times. The role involves follow up and of patients and their families who have been discharged from the Neonatal Unit, either in their homes or in our specially set up Outreach clinics.

We are also proud to host the Defence Medical Group South East at Frimley Park with military surgical, medical and nursing personnel working alongside the hospital’s NHS staff providing care to patients in all specialties
To maintain adequate and appropriate records of patients and supply information as required in line with Trust Policy.
To develop policies and protocols alongside team members to enhance delivery of effective patient care and to adhere to NMC guidelines, and Network and Trust policies.
Have sound knowledge of clinical governance issues, promoting evidence-based practice.
To assist in maintaining safety of personnel, environment and equipment.
To act as a mentor/role model for students and staff, taking them out on the community for work experience
To assist with the teaching/training of both hospital/community staff involved in Neonatal care. Aid the professional development of team members and within the multidisciplinary team as appropriate.
To highlight social and safeguarding concerns and refer as necessary. To participate in case conferences and other safeguarding meetings. To have sound knowledge of child safeguarding procedures.
Act as a resource for members of the multidisciplinary team.
Empower parents and carers to enable them to safely care for their child within the community environment through education, support and risk assessment
