Description:

The post holder will be responsible for leading the Uttlesford North Integrated team, who through partnership with members of the Community Nursing team, will ensure high quality evidence based care to patients, their families and carers in the Community. The post holder will also be responsible for providing leadership, line management and clinical supervision along with the day to day management of the integrated service team as well as having a clinical oversight of the Saffron Walden outpatient department. They will work collaboratively with the South Uttlesford Team lead, ensuring cohesive leadership across both North & South, whilst remaining aligned to their own Primary Care Networks (PCN).
The post holder will work closely with the North Uttlesford Integrated PCN and will be an integral part of the West Essex Care Closer to Home model, working in partnership with Social Services and other statutory and non-statutory services to meet the needs of the local population using public health information/population management to support service development. In addition they will work closely with the Virtual Hospital and Care Co-ordination Centre (CCC) to prevent hospital admission, facilitate hospital discharges and ensure care is delivered in the right place at the right time.

To work within a Caseload Management Framework and ensure coordination of care for patients identified through collaborative working with the wider Primary Health Care team and through clinical judgement.
To have clinical oversight of the Outpatients Department in Saffron Walden Community Hospital
To identify and plan preventative measures and anticipatory care needs.
To maintain accurate contemporaneous and up-to-date patient records and inform other appropriate professionals about changes in conditions
To proactively monitor patients and review care plans
To make referrals to other services or professions, request diagnostic tests and instigate treatments and interventions as required
To assess patients with complex clinical need using clinical examination skills, history taking and evaluation skills.
To manage End of Life Care and work in partnership with health, social and voluntary agencies to ensure patients are treated with dignity and respect and to support their families and carers.
To prepare the patient and their family/carer for changes in the patient’s condition and support choice for end of life care
