Description:

Work within the practice to provide and monitor care, alongside the wider MDT, contributing to the delivery of excellent patient care, supporting the existing workforce under direct or indirect supervision. Improve safety and quality of care at every opportunity. Undertake additional clinical training as required by the practice to support the QOF/Network Contract/DES Services Specification. Support registered nurses and the wider team to enable them to focus on the more complex clinical care.
To support with chronic disease clinics and maintaining registers, deliver patient care, referring patients to other members of the MDT or to secondary/specialist care as appropriate. To implement and evaluate individual treatment plans dependant on training and experience. Provide support and supervision to trainee nursing associates, healthcare assistants, apprentices, and those on learning placements appropriate to level of competence. Accurately perform and record clinical observations such as blood pressure, temperature, respiration, and pulse rates.
Engage positively with patients/carers/families and staff members promoting mutually respectful and appropriate relationships. Communicate effectively with patients regarding providing and planning their care and carrying out risk assessment needs as required. Communicate complex and sensitive information verbally and in writing. Adapt and tailor communication skills to the need of the listener.
To act as patient advocate during practice meetings, sharing any relevant information confidently and accurately, maintaining patient confidentiality appropriately. Develop an understanding of MDT structures and members roles. Support patients to improve and maintain their physical, mental, and behavioural health and well-being. Provide advice to patients including promoting self -management where appropriate and signposting patients to other primary care colleagues, local community, and voluntary sector services Maintain accurate and contemporaneous patient health records.
Work in adherence to NMC Code for Nursing Associates, within own level of competence and the protocols and policies of the practices/PCN. Work flexibly to meet the needs of the service and patients. Adapt to what may be potentially challenging and changing situations at times. Willing to undertake other duties deemed appropriate to a Nursing Associate following correct training/guidance and agreed by the post holder and their line manager.
The post-holder will participate in training programmes implemented by the practice as part of this employment. This will include: Participation in an annual individual performance review, including taking responsibility for maintaining a record of own personal and/or professional development. Taking responsibility for their own development, learning and performance. They may be involved in demonstrating skills and activities to others who are undertaking similar work.
The Nursing Associate must continue with revalidation in accordance with NMC guidelines Other Responsibilities within the Practice Awareness of and compliance with all relevant practice policies/guidelines, e.g. prescribing, confidentiality, data protection, health and safety A commitment to life-long learning and audit to ensure evidence-based best practice Contributing to evaluation/audit and clinical standard setting within the organisation Work as an effective and responsible team member, supporting others and exploring the mechanisms to develop new ways of working Provide an educational role to patients, carers, families and colleagues in an environment that facilitates learning. Read-Coding patient data ensuring the highest level of data quality. Attending training and events organised by the practice or other agencies, where appropriate.
Complying with all practice protocols in relation to GP practice administration and clinical practice. Adhere to the relevant NHS Codes of Practice and own professional Code of Conduct.
