Help

• Serve as a Clinical Nurse in the telehealth/telemedicine program with duties performed in an ambulatory care setting.
• Use interactive audio-video communication technologies to assist remote licensed independent providers and supporting clinical and administrative staff.
• Provide independent, comprehensive nursing care on-site and remotely to expand access to multifaceted acute, routine and specialty care consultation services.
• Assist with patient and staff education.
• Coordinate patient direct and network referrals, discharge instructions and telephonic follow-up upon the conclusion of the telehealth/telemedicine visit to ensure continuity of care across the clinical continuum.
• Evaluate the quality of data transmission and interactions during the telehealth/telemedicine encounter to support and optimize the capacity to examine, diagnose and develop an appropriate plan of care.
• Participate in hospital and clinic meetings and performance improvement activities as assigned.
