Responsibilities:

We are looking for a Nurse Home Visitor to join our team in the Family Partnership Program (FPP) at Congress.
The Australian Nurse Family Partnership Program (FPP) is a national nurse home visitation program that has been rigorously evaluated for over 25 years.
The program involves Registered Nurses and/or Midwives working in partnership with Aboriginal Family Partnership Workers and enrolled clients who are pregnant with an Aboriginal baby from early in their pregnancy and continues until the child is 2 years old, through an intensive culturally responsive home visiting program.
The Congress Nurse Home Visitor and Aboriginal Family Partnership Workers develop a long-term relationship with clients that builds client capacity, skills, confidence to ensure self-determination with the ability to determine their own future.
We are seeking an enthusiastic strengths-based practitioner who is interested in teamwork and building warm therapeutic relationship that support families to be the best parents they can be.
