Description:

This position presents an exciting opportunity to work with key members of our healthcare team and collaborate with other Nurses, healthcare professionals (including GPs, physiotherapists, dieticians, occupational therapists, social workers etc. ) and to shape the service delivery of Signature Care into the future during a period of significant change in the aged care sector.
We are looking for an experienced Nurse Practitioner with the experience, expertise and authority to diagnose and treat our residents with a variety of acute or chronic health conditions. Someone that can assist in the process of continuous improvement by assisting to reduce the number of hospitalisations, provide appropriate referrals to specialists, prescribe medications, monitor care recipients that are on a wellness program, undertake clinical research in the wellbeing program by collecting and analysing data and following this up with general practitioners, as well as policy development.

SKILLS & EXPERIENCE REQUIRED:

  • Ideally, at least 3 years’ experience in a similar role
  • Excellent interpersonal, communication & leadership skills
  • Experience in working with and leading a multidisciplinary team
  • Strength in building customer and stakeholder relationships
  • An understanding of the Medicare Benefits Schedule
  • An understanding of the Department of Health regulations for residential Aged Care
  • Current AHPRA registration
  • Previous experience in an aged care background is desirable
